Luis Vernaza Hospital of Guayaquil. Period March to September 2014. The instrument used for the preparation of this research project is the survey of nurses in the cardiology ward and patients readmitted after a cardiovascular surgery due to complications arising in the home. According to research reveals that not all nursing staff perform direct patient care who graduate from cardiovascular surgery, therefore, patients say they were not educated about the activities to be performed at the time of discharge and at home. The rate of hospital readmissions much depends on the role played by nurses during the patient's stay even more at the time of discharge, therefore, the most common complications that occur at home are pain in the area Chest around the incision, swelling in the leg where the vein graft and trouble it took to breathe, to this, a discharge plan for staff nurses to be applied to all patients who graduated in cardiovascular surgery is performed , thus, decrease the rate of hospital readmissions.
